Can Video On Demand and PayPerView be initiated from a TiVo with CableCARDs

Regarding this quote from this website:

"This means that Video On Demand and PayPerView cannot be initiated from a TiVo with CableCARDs. But if the programming is unlocked on the cable account (by phone, online, or from a cable box on the same account) then the TiVo's CableCARDs will generally be able to view and record those shows."

Is it possible to have the programming unlocked on the cable account? Are cable customers ever able to achieve this?

Unlocked on an entire account? Per show, yes, all the time, using one of the methods described above.

Been here a long time . . .

I guess from your reply it cannot be done for the entire account but only show by show? How would I do it online?

What do you mean by "the entire account?" Can you be more specific? On-demand programming is necessarily show-by-show, right?
