Need help identifying a Model Type

I have a very old TiVo DVR which has exhibited frequent macro-blocking and pixellation. I presume this indicates the hard drive is failing and should be replaced. There is no model number on the back; instead it reads "CE Serial No. PROT 0063". This may be a prototype model. The front silver bezel has no other logo than a TiVo logo, but has several operative buttons arranged in a sort of "swoosh" design. Does anyone know what model this is and what hard drive I need to order?

Can you post the exact TiVo OS version from the System Information screen? Then we should be able to help.
